WAIVER AGREEMENT

Drama is a physical activity so there is always a risk of injury, including serious injury. We will do everything possible to mitigate these risks and provide a safe and secure environment but we ask parents to acknowledge this risk, and sign to confirm that all activities are undertaken with the knowledge of this risk:

I, the legal guardian or parent of the child listed above, hereby agree to irrevocably release and waive any claims that I or my child now or hereafter may have against Danielle Gaglione and Dramarama.

PHOTOGRAPH AUTHORISATION *

We may take photos and videos of the children, to share with them during class, with you, the parents after class and also to be shared on our website or social media platforms. Please therefore let us know if you are happy with this:

1. No I do not want my child to be photographed or videoed

2. My Child can be photographed or videoed but images are not to be shared online

3. My Child can be photographed or videoed and images can be shared on social media or on the website (we never publish names alongside images)
